青年人胃癌

本文报告沂蒙山区4所医院近年来诊治的43例青年人胃癌。男18例,女25例。占同期胃癌住院病人的3.4%。手术治疗40例,病灶切除率为62.5%(25/40)。其临床特点是早期症状缺乏特异性;女性多;胃窦部癌睡多发(占74%);恶性程度高,粘液癌与实体癌占76%;病程发展快,转移早,预后差,术后5年存活仅1例。提高青年人胃癌诊治水平需(1)提高对青年人胃癌的认识;(2)对青年人“胃病”作随访追踪;(3)可疑病例作胃镜检查。

GASTRIC CARCINOMAS IN YOUNG PATIENTS

This paper reports 43 cases of gastric carcinomas in adults below 30, of whom 18 cases are males and 25 cases are females,constituting 3.4% of all patients in the same period.All of the 43 cases were diagnosed in the early time as patients of gastric ulcer,chronic gastrititis or chole- cystitis.Forty cases were surgically treated.The focus resectability rate was 62.5%.The clinical characteristics are as follows:The early sym- ptoms were lacking specificities;There were more females among the cases;There were more carcinomas of the gastric sinus,constituting 74%; The carcinomas were highly maglinant,mucous carcinomas and hard car- cinomas constituting 76%.The course of disease developed rapidly and the prognosis was poor with only one case existing for 5 years after the operation.To raise the level of diagnosis and treatment of gastric carci- nomas in young patients,physicians should:firstly,acquire a deeper understanding of the gastric carcinomas in young patients;secondly,make a follow-up survey of the gastric disease of young patients and thirdly, examine every doubtful case with an endoscope.
