乳腺导管内增生性病变及浸润性癌中树突状细胞的检测

摘    要:
目的 检测乳腺导管内增生性病变和微小浸润性癌及浸润性导管癌中树突状细胞(dendriticcells,DC)和T细胞的 分布状况和浸润密度,探讨乳腺癌发生、发展中机体免疫状态的变化规律。方法 应用免疫组化S P法和两步法对16例正常 副乳腺、58例导管内增生性病变、4例微浸润导管癌及67例浸润性导管癌乳腺标本进行S 100蛋白+DC、HLA DR+DC、CD1a +DC(DC三参数)及CD45RO+T细胞的浸润密度检测。结果 浸润癌中DC三参数浸润密度均高于其它病变组织(P< 0.05)。导管原位癌、微浸润癌及浸润癌组织中CD45RO+T细胞浸润密度均高于其它病变组织(P<0.05)。DC三参数间及 其与CD45RO+T细胞间均呈正相关(P<0.001)。结论 DC浸润密度随增生性病变加重而逐渐增高,发展为浸润癌时则明 显增高。

Abstract:
Purpose To examine the distribution and infiltrating density of dendritic cells(DC) and T lymphocytes in intraductal hyperplastic lesions,micro-infiltrative carcinoma and infiltrating ductal carcinoma of breast. Methods Immunohistochemical staining for S-100 protein,HLA-DR,CD1a,and CD45RO was performed in 145 routinely paraffin embedded specimens,including 16 cases of mamma accessoria,58 cases of intraductal hyperplastic lesions,4 cases of micro-infiltrative carcinoma(MIC),and 67 cases of infiltrating ductal carcinoma(IDC). The infiltrating densities of S-100 protein+DC,HLA-DR+DC,CD1a+DC (three parameters labeling DC),and CD45RO+T lymphocytes were investigated. Results All infiltrating densities of three parameters labeling DC in IDC were significantly higher than those in other cases,including mamma accessoria,intraductal hyperplastic lesions,and MIC(P<0.05). Infiltrating densities of CD45RO+T lymphocytes were significantly higher in cases of DCIS,MIC and IDC than in other intraductal hyperplastic lesions(P<0.05). Positive correlations among infiltrating densities of three parameters labeling DC and CD45RO+T lymphocytes were showed significantly (P<0.001). Conclusions Infiltrating densities of DC show an increasing trend from normal breast to infiltrating ductal carcinoma,and a significant increase was showed in IDC.
