糖尿病视网膜病变的中医证候要素研究

目的:运用因子分析法探讨糖尿病视网膜病变(diabetic retinopathy,DR)的中医证候要素及其分布规律。方法:采集4个研究单位诊治的929例DR患者的四诊指标,通过因子分析法对事先未经经验辨证的四诊指标提取证候要素。结果:得到23个公因子(即证候要素),经合并后,初步诠释为阴虚、气虚、血瘀、阳虚、痰、血虚、精亏、气滞、湿、热(火)10种病性类证候要素,其中阴虚、气虚、血瘀、阳虚位居前4位,为DR的多见证候要素,此与中医关于DR病因病机的认识基本一致。结论:因子分析是确定DR常见证候要素的有效方法,本文提取的证候要素,可作为DR辨证论治的重要参考。

Study on the Syndrome Elements of Traditional Chinese Medicine about Diabetic Retinopathy

Objective:To discuss the TCM syndrome elements and their distribution rule of diabetic retinopathy (DR) through using the factor analysis method.Methods:We collected four diagnostic indicators of 929 patients from four research institutes with DR and extracted the syndrome elements of four diagnostic indicators without syndrome differentiation in advance through using the factor analysis method.Results:We got 23 common factor(the syndrome elements) after study and these syndrome elements were interpreted preliminarily as 10 kinds of disease syndrome elements which were respectively Yin deficiency,Qi deficiency,blood stasis,Yang deficiency,phlegm and blood deficiency,deficiency of essence and Qi stagnation,wet,hot(fire) after economic cooperation.Among them,Yin deficiency,Qi deficiency,blood stasis and Yang deficiency were in the top four respectively and were the syndrome elements of DR.The results were basically identical with the understanding of TCM about etiology and pathogenesis of DR.Conclusion:Factor analysis is an efficient method to determine the syndrome elements of DR and the syndromes elements extracted in this paper could be the important reference for syndrome differentiation and treatment of DR.
