慢性心力衰竭患者中医证候要素相关性研究

目的:运用回顾性研究法研究慢性心力衰竭患者中医证候要素的构成及其之间的相关性。方法:采用回顾性调查的方式,对慢性心力衰竭住院患者的中医证候进行调查,并进行统计分析。结果:气虚、阳虚、阴虚、痰浊、水饮、血瘀是心力衰竭的基本病性要素,心、肺、脾、肾、肝是基本病位要素;心力衰竭的病位证素在不同证候中具有对应的病性类证素。在气虚、痰浊为主的证候中,多与肺相关;在阳虚、血瘀为主的证候中,多与心相关;而以水饮为主的证候中则与心、肺均相关。结论:通过对心力衰竭证候要素及其相关关系的研究,为建立以证候要素为基本单位的辨证新体系提供理论依据,并为心力衰竭临床辨证提供新的思路,提示临床辨证可灵活组合应用。

The Correlation Study on TCM Syndrome Elements of Patients with Chronic Heart Failure

Objectives:To study the composition of TCM syndrome elements in patients with chronic heart failure and the correlation between them by retrospective methods.Methods:TCM syndrome of chronic heart failure hospitalized patients were investigated and analyzed by retrospective study.Results:Qi deficiency,Yang deficiency,Yin deficiency,turbid phlegm,excessive fluid and blood stasis were basical elemens of nature of disease.Heart,lung,spleen,kidney and liver were basical elemens of location of disease.The elements of location of disease have corresponding elements of nature of disease in different syndrome types.The syndromes of Qi deficiency and turbid phlegm have correlation with lung.The syndromes of Yang deficiency and blood stasis have correlation with heart.The syndromes of excessive fluid have correlation with lung and heart.Conclusion:The correlation study on TCM syndrome elements of patients with chronic heart failure can provide theoretical basis for the construction of new syndrome differentiation system with syndrome element as basic unit.The study can also provide new way of thinking and prompt flexible combined application for clinical differentiation of heart failure.
