疏肝和胃汤治疗老年慢性萎缩性胃炎临床研究

目的:观察疏肝和胃汤治疗幽门螺杆菌(helicobacter pylori,Hp)阳性老年慢性萎缩性胃炎临床疗效及对胃镜组织学的影响。方法:76例Hp阳性老年慢性萎缩性胃炎患者随机分为对照组38例和观察组38例。对照组予以三联疗法治疗,观察组在对照组治疗基础上给予疏肝和胃汤治疗。比较两组患者临床疗效、临床症状积分及胃镜下观察。结果:对照组有效率为73.68%,观察组有效率为92.11%,两组比较,差异有统计学意义(P0.05);与治疗前比较,两组治疗后主要症状积分降低,治疗后胃镜观察分级降低,治疗后腺体萎缩分级降低,治疗后肠上皮化生分级降低,差异均有统计学意义(P0.05);与对照组治疗后比较,观察组治疗后主要症状积分较低,治疗后胃镜观察分级较低,治疗后腺体萎缩分级较低,治疗后肠上皮化生分级较低,差异均有统计学意义(P0.05)。结论:疏肝和胃汤治疗Hp阳性老年慢性萎缩性胃炎疗效显著,可以逆转腺体萎缩、肠化生。

Clinical Study on Shugan Hewei Tang in the Treatment of Senile Chronic Atrophic Gastritis

Objective:To explore the clinical effect and endoscopic histology of Shugan Hewei Tang in the treatment of senile chronic atrophic gastritis with Hp positive.Methods:76 cases with senile chronic atrophic gastritis with Hp positive were randomly divided into the control group and the observation group,each of 38 cases.The control group was given triple therapy,and the observation group was treated with Shugan Hewei Tang based on the control group.Clinical symptom relief,comparative clinical efficacy and endoscopic histology were compared after treatment.Results:The effective rate in the control group (73.68％) was lower than the study group (92.11％),with significant difference (P ＜ 0.05),and compared with before treatment,main symptom score decreased,gastroscopic observation grade decreased,glandular atrophy grade decreased,intestinal metaplasia grade decreased,all with statistical significance(P ＜ 0.05).Compared with the control group,main symptom score in the study group was lower,gastroscopic observation grade was lower,glandular atrophy grade was lower,intestinal metaplasia grade was lower,all with statistical significance(P ＜ 0.05).Conclusion:Shugan Hewei Tang in the treatment of elderly patients with chronic atrophic gastritis with HP positive is effective,which can reverse the gland atrophy,intestinal metaplasia.
