100例不稳定心绞痛临床分析

目的探讨低分子肝素应用于不稳定性心绞痛的安全性和有效性。方法将2009年1月～2009年9月入住我院心血管病房的100例患者随机分为试验组（50例）和对照组（50例）,对照组常规抗心绞痛治疗加用普通肝素,治疗组在常规抗心绞痛治疗的基础上加用低分子肝素钠,观察比较两组的疗效及不良反应。结果临床疗效治疗组总有效率为94.0%,对照组总有效率78.0%,两组比较有显著性差异（p〈0.05）。治疗组的心电图改善情况与对照组相比,差异具有统计学意义（p〈0.05）。两组血小板聚集率、全血黏度、血浆黏度显著降低（p〈0.01）,治疗组与对照组比有差异（p〈0.05）。治疗过程中,对照组有1例患者穿剌部位渗血,1例患者出现皮下瘀斑,2例患者牙龈出血;治疗组有仅有1例出现注射部位皮下瘀斑,停药后逐渐好转。结论不稳定型心绞痛的治疗上低分子肝素的疗效明显高于普通肝素,并且出血危险性方面优于肝素组,值得推广应用。

Clinical analysis of 100 cases of unstable angina pectoris

Objective To explore the safety and effectiveness of the low molecular weight heparin applied to unstable angina pectoris.Methods During January 2009 to September 2009,100 cardiovascular patients staying at our hospital wards were randomly divided into trial group（50 cases） and control group（50 cases）.The control group therapy was treated by routine anti-angina with ordinary heparin and the treatment group was treated by the routine anti-angina therapy plus low molecular weight heparin on the basis of observed,comparing the efficacy and adverse reactions of the two groups.Results The total effective rate was 94.0% in the control group,the total efficiency rate was 78.0%,two groups had significant difference（p 〈0.05）.The difference between improvement situation of ECG was statistically significant（p〈 0.05）.Two groups of platelet aggregation,whole blood viscosity,plasma viscosity significantly lower（p〈0.01） than before.Treatment group and control group are different（p 〈0.05）.The course of treatment,the control group included 1 patient puncture site bleeding,1 patient subcutaneous ecchymosis and gum bleeding of 2 patients;the treatment group,only 1 patient subcutaneous ecchymosis at the injection site,the gradual improvement after stopping treatment.Conclusions The treatment of unstable angina effect of low molecular weight heparin was significantly higher than unfractionated heparin,and the risk of bleeding than heparin in terms of groups which is worthy of popularizing.
