摘    要:目的研究凝固酶阴性葡萄球菌(CNS)抗药基因携带及其对氯己定类消毒剂抗力变化情况。方法采用细菌分离鉴定方法和基因扩增技术,对医院环境和医务人员手上采样标本进行了分离培养和检测。结果在841份标本中,检出63株CNS,检出率为7.49%。63株CNS中,有12株检出qacA基因,检出率为19.05%。醋酸氯己定对12株CNS最小抑菌浓度范围在0.24 mg/L~1.96 mg/L,其中有7株CNS的最小抑菌浓度高于标准菌株者;最小杀菌浓度范围在7.81 mg/L~500 mg/L,其中有9株CNS的最小杀菌浓度明显高于标准菌株。结论医院环境表面和医务人员手上CNS检出率较高,多数携带qacA基因的菌株对醋酸氯己定消毒剂抗力有增加的趋势。

Abstract:Objective To study the resistance change of coagulase negative Staphylococcus(CNS) carrying drug-resistant gene to the chlorhexidine disinfectant.Methods Bacterial isolation and identification method and gene amplification techniques were used to detect sample specimens from the hospital environment and medical personnel′ hands.The minimum inhibitory concentration test was used to observe disinfectant resistance of CNS.Results From 841 shares specimens,63 strains CNS were detected and the positive rate was 7.49%.From 63 strains CNS,12 strains CNS carrying qacA gene were detected and the positive rate was 19.05%.The minimum inhibitory concentration of chlorhexidine acetate to 12 strains CNS ranged from 0.24 mg/L to 1.96 mg/L and the MICs of 7 strains CNS were higher than those of the standard strain;The minimal bactericidal concentration ranged from 7.81 mg/L to 500 mg/L and the MBCs of 9 strains CNS were significantly higher than those of the standard strain.Conclusion The detection rate of CNS from hospital environmental surfaces and medical personnel' hands is comparatively high and the resistance of majority of strains carrying qacA gene to chlorhexidine acetate disinfectant has an increasing trend.
