非小细胞肺癌患者 D-二聚体、凝血指标和生存预后的相关研究

目的：探讨非小细胞肺癌患者的 D-二聚体、凝血指标与患者生存时间的关系。方法选择承德医学院附属医院2010年1月至2011年3月收治的初治的非小细胞肺癌患者共57例，记录患者治疗前 D-二聚体及凝血指标，对患者进行随访，总随访期限为36个月。对数据进行整理、汇总及统计分析，分别进行单因素生存分析和多因素生存分析。结果单因素生存分析显示 D-二聚体升高、纤维蛋白原升高、凝血酶原时间升高及疾病分期晚是使总生存期缩短的主要因素。采用 Cox 回归模型进行多因素生存分析显示 D-二聚体升高、凝血酶原时间升高及疾病分期晚是非小细胞肺癌患者生存期缩短的独立影响因素。结论 D-二聚体升高、凝血酶原时间升高、疾病分期晚是影响非小细胞肺癌患者总生存期的独立因素。

Correlation study of D-dimer,coagulation indexes and survival time in patients with non-small cell lung cancer

Objective To explore the relationship of D-dimer,coagulation indexes and the survival time in patients with non-small cell lung cancer.Methods 57 patients with non-small cell lung cancer accepting early treatment in Affiliated Hospital of Chengde Medical College from January 2010 to March 201 1 were collected.D-dimer and coagulation index were recorded before treatment.The patients were followed up for 36 months.Data were made sort,summary and statistical analysis including univariate and multivariate survival analysis.Results Univariate analysis showed elevated D-dimer,increased fibrinogen, increased prothrombin time (PT) and late stage of the disease were main factors of shortened overall survival.Cox regression model for multivariate survival analysis showed that elevated D-dimer,increased PT and late stage of the disease were the independent factors of survival period shorten in patients with non-small cell lung cancer.Conclusions The elevated D-dimer,increased PT and late stage of the disease are independent factors of overall survival in patients with non-small cell lung cancer.
