3种方法对儿童幽门螺杆菌感染检测的比较

目的对大便幽门螺杆菌(Hp)抗原检测方法、血清Hp抗体检测方法及13 C尿素呼气试验3种方法的灵敏度、特异性和诊断符合率进行比较,为儿童Hp感染检测适宜方法的选择提供依据。方法采集64例Hp感染患儿大便及血清分别进行大便Hp抗原、血清Hp抗体检测,并同时进行~(13)C尿素呼气试验。结果大便Hp抗原检测、血清Hp抗体检测及~(13)C尿素呼气试验的灵敏度分别为95.9%、95.6%、97.9%;特异性分别为93.3%、77.8%、93.8%;诊断符合率分别为95.3%、90.6%、96.9%。结论 ~(13)C尿素呼气试验及大便Hp抗原检测对儿童Hp感染的灵敏度、特异性及诊断符合率均较高,其中大便Hp抗原检测对儿童依从性更好,适于儿童Hp感染的检测。

Comparison of three methods for the detection of Helicobacter pylori infection in children

Objective To compare sensitivity ,specificity and diagnostic coincidence rate of stool Helicobacter pylori(Hp) antigen method ,serum Hp antibody method and 13 C urea breath test in the detection of Hp infection for children ,and provide basis for choosing suitable detection methods .Methods 64 children with Hp infection were enrolled in the study ,whose stool and blood samples were collected and tested for Hp antigen and antibody respectively ,at the same time those children also underwent13C urea breath test .Results For stool Hp antigen method ,serum Hp antibody method and 13C urea breath test ,the sensitivity were 95 .9% ,95 .6% ,97 .9% respectively ,the specificity were 93 .3% ,77 .8% ,93 .8% respectively ,the diagnostic coincidence rates were 95 .3% ,90 .6% ,96 .9% respectively .Conclusion 13 C urea breath test and stoGol Hp antigen method have higher sensitivity ,speci‐ficity and diagnostic coincidence rate ,the stool antigen method have better compliance ,which is suitable for the detection of Hp in‐fection in children .
