首发脑卒中病人生活质量现状及其影响因素分析

[目的]探讨首发脑卒中病人生活质量现状，并了解其影响因素，为今后全面提高脑卒中病人的生活质量提供依据。[方法]采用自行设定的一般资料问卷，Barthel指数量表，慢性病自我效能量表，脑卒中特定生活质量量表对156例首发脑卒中病人进行问卷调查。[结果]首发脑卒中病人的生活质量平均得分为170分，明显低于总分245分，处于较低水平。方差分析显示，病人的医保类型、体重指数、性别、烟酒史、初发就诊时间、自我效能水平以及日常生活活动能力（ADL）对生活质量的影响有统计学意义。多元线性回归分析结果显示，影响脑卒中病人生活质量的主要因素为病人的自我效能水平和ADL（P〈0．05）。[结论]脑卒中病人的生活质量处于较低水平，且影响生活质量的因素包含多个方面。临床可通过多种方式联合干预，能更有效改善脑卒中病人的生活质量。

Analysis on quality of life and its influencing factors of starting stroke patients

Objective.To probe into the status quo of quality of life and its influencing factors in patients with first stroke, so as to provide the evidences for improving patients＇ quality of life totally. Methods. A to- tal of 156 starting stroke patients were investigated by using self - de- signed general information questionnaire, Barthel index scale, chronic disease self - efficacy scale, stroke - specific quality of life scale. Results. The average score of quality of life of starting stroke patients was 170, which was much lower than total score 245. Variance analysis showed that the influence of patient＇s health insurance type,body mass index, gender, tobacco and alcohol history, onset to treatment time,self - efficacy levels and ADL were statistically significant for their quality of life. Multiple linear regression analysis showed that the main influencing factors were self- efficacy and ADL for quality of life of stroke patients （P〈0.05）. Conclusion.The quality of life of starting stroke patients is at lower level, which is influenced by mixed factors. The joint interven- tions in a variety of ways should be taken in clinic so as to improve pa- tients＇ quality of life effectively.
