清洁灌肠与心理护理联合中药保留灌肠治疗溃疡性结肠炎随机平行对照研究

目的]观察清洁灌肠与心理护理联合中药保留灌肠治疗溃疡性结肠炎疗效。方法]使用随机平行对照方法,将40例住院患者按随机数字表法为两组。对照组20例四君子汤合葛根黄芩黄连汤保留灌肠(党参、白术、茯苓、葛根、黄芩、黄连各15g,甘草8g;气虚乏力加黄芪30g;寒者加附子12g;五更泄泻加四神包煎20g;腹痛腹胀加元胡、川朴、木香各10g);药液温度为37℃,100mL~150mL/次,药液停留时间尽可能长,1次/d。治疗组20例生理盐水500mL~800mL反复多次进行不保留灌肠,直至排出液清洁无粪质为止;心理护理:治疗前护理人员应向病人讲解中药灌肠作用及配合方法,解除病人心理负担,以取得病人积极配合;创造安静环境,保护病人隐私,冬天室温26℃~28℃,灌肠前嘱病人排空大小便;中药液保留灌肠同对照组。连续治疗30d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。结果]治疗组显效6例,有效12例,无效2例,总有效率90.00%。对照组显效1例,有效9例,无效10例,总有效率50.00%。治疗组疗效优于对照组(P0.01)。结论]清洁灌肠与心理护理联合中药保留灌肠治疗溃疡性结肠炎效果显著,值得推广。

Results ] The treatment group 6 cases had marked effect, 12 cases effective, ineffective in 2 cases, total effective rate was 90.00%. Effective 9 cases, control group 1 case had marked effect, 10 had no effect, the total effective rate was 50.00%. The treatment group curative effect is better than that of control group （P〈0.01） . Conclusion ] cleaning enema, psychological nursing combined traditional Chinese medicine retention enema in treatment of ulcerative colitis effect is remarkable, is worth promoting.
