卵巢和胃粘液癌CK 7、CEA和CK 20单克隆抗体的表达

目的：研究细胞角蛋白7（CK7）、癌胚抗原（CEA）和细胞角蛋白20（CD20）单克隆抗体在卵巢和胃粘液癌中的表达及鉴别意义。方法：采用免疫组织化学S-P法对各组癌进行3种单克隆抗体表达的检测。结果：二组间各种抗体染色有一定的重叠。8例（1*）来源于卵巢的粘液癌CK7均呈阳性表达“ ”-“ ”，CEA3/8不同程度表达，CK20 7/8阴性，经实验结果证实其中1*例应是原发于卵巢；18例胃来源的粘液癌CK7、CEA和CK20皆不同程度阳性。经t检验，CK7和CEA表达在两组间有显著差异（P＜0.05)，而CK20在两组间表达无显著差异（P＞0.05)。结论：单一CK7、CEA和CK20不能有效鉴别来自胃和卵巢的粘液癌。CK7 CEA CK20联合是鉴别卵巢来源和胃来源粘液癌最有效的组合。

The expression of cytokeratin 7,carcinoembryonic antigen,cytokeratin 20 in the mucinous ovarian carcinoma and mucinous gastric carcinoma

Objective　To discriminate if the mucinous carcinomas are primary to the ovary,stomach or metastases from ovary and stomach by monoclonal antibodies cytokeratin 7,carcinoembryonic antigen,and cytokeratin 20.Methods　Immunohistochemical methods were used with these monoclonal antibodies and S-P kit.Results　The immunostaing patterns of CK 7,CEA,and CK 20 showed a considerable overlap between the two types of mucinous carinomas,8 cases of ovarian mucinous carcinomas were always positive for CK 7 and a lesser extent for CEA,18 cases of gastric mucinous carcinomas showed a different degree positivity for CK 7,CEA,and CK 20.In discriminant analysis,three antibodies (CK 7,CEA,CK 20) appeared to be necessary for optimal classification,7/8 ovarian mucinous carcinomas and 10/18 gastric mucinous carcinomas were classified correctly,and 1 case of gastric mucinous carcinoma should originate from ovary.The difference of CK 7 and CEA between the two groups was significant(P<0.05),but the difference of CK 20 between the two groups was not significant(P>0.05).Conclusion　One of CK 7,CEA,and CK 20 may not differentiate between the two groups.The combination of CK 7,CEA,and CK 20 is sufficient for classification in most mucinous carcinomas originated to stomach or ovary.
