肝细胞癌中卵圆细胞表面标记的表达及意义

摘    要:
目的探讨卵圆细胞部分表面标记在肝细胞癌中的表达状况和意义。方法应用免疫组织化学SABC法检测20例肝细胞癌中CK7、CK19、CK18、CD34、e—kit抗原的表达。结果14例肝细胞癌CK7表达呈阳性,其阳性表达率为70%(14/20);9例c—kit的阳性表达率为45%(9/20);在20例肝细胞癌癌组织中的CK18全部呈阳性表达,CK19和CD34全部呈阴性表达。CK7、C—kit表达与患者年龄、性别无关,与肿瘤大小、是否合并肝硬化以及组织学分级也不相关(P〉0.05)。结论肝细胞癌癌组织中存在表达卵圆细胞表面标记的细胞,其表达CK7、C—kit,不表达造血干细胞标记物CD34。其进一步提示,肝细胞癌可能不是起源于肝干细胞而是来源于过渡性的前体细胞/祖细胞,如来源于Hering管的卵圆细胞。

Abstract:
Objective To identify the expression of surface markers on oval cells and explore its implications in hepatic cellular carcinoma(HCC). Methods Surface markers of oval cells (CK7, CK18, CK19, CD34 and c - kit) antigen was tested using histochemical method (SABC) in 20 cases with hepatic cellular carcinoma. Results Expression of CK18 was positive in all 20 hepatic cancer samples. Expression of CK19 was not observed in all hepatic cancer samples except in the sample of epithelial cell of small bile duct and expression of CD34 was not observed in all hepatic cancer samples. CK7 expression was positive in 14 HCC samples ( 14/20, 70% ) and c - kit expression was positive in 9 samples (9/20, 45% ). Expression of CK7 and c -kit were not found in 4 samples of parenchyma cells of normal hepatic tissues. The expression of CK7 and c - kit is not related to the age, gender of patients, neither to compilation of hepatocirrhosis or histological classification ( P 〉 0.05 ). Conclusion In HCC tissues, there are cells that can express surface markers of oval cells. These cells can express CK7 and c -kit, but cannot express CD34, the marker of hematopoietic stem cell. This suggests that HCC may not be originated by hepatic stem cell, but rather by transitional progenitor cell, such as the oval cell originated from Hering duct.
