肠易激综合征中医证型与胃肠激素水平变化的相关性研究

目的观察肝郁气滞型、脾胃虚弱型、肝郁脾虚型肠易激综合征患者胃肠激素的变化，探讨肠易激综合征的中医证型与血浆胃肠激素水平变化的相关性。方法103例肠易激综合征患者分为肝郁气滞证组35例，肝郁脾虚证组38例，脾胃虚弱证组30例，正常对照组10例。运用放射免疫同批测定血浆胃动素（MOT）、胆囊收缩素（CCK）和生长抑素（SS）水平。结果脾胃虚弱证组患者血浆MOT水平显著高于正常对照组（P〈0．05）；肝郁气滞证组明显低于正常对照组（P〈0．05）。3个证型组间比较，脾胃虚弱证组MOT水平显著高于肝郁气滞证组（P〈0．01）和肝郁脾虚证组（P〈0．05）。3个证型组血浆SS水平明显高于正常对照组（P〈0．01，P〈0．05），其中肝郁气滞证组明显高于脾胃虚弱证组和肝郁脾虚证组（P〈0．05）。结论不同中医证型的肠易激综合征患者上述胃肠激素存在不同的变化。肠易激综合征的中医证候与胃肠激素水平的变化具有相关性。

Correlation between TCM Syndrome Type and Gastro-intestinal Hormones Level in Patients with Irritable Bowel Syndrome

Objective To observe the change of gastro-intestinal hormone levels about syndrome type of stagnation of liver qi, deficiency of spleen and stomach, liver qi stagnation and spleen qi deficiency in patients with irritable bowel syndrome （IBS）, and to probe into the correlation between TCM syndrome type and gastro-intestinaI hormone levels. Methods One hundred and three cases of patients with IBS were divided into liver qi stagnation group （35 cases）, spleen and stomach deficiency group （30 cases） and liver qi stagnation and spleen qi deficiency group （38 cases）, with normal group （10 cases） as control. Plasma motilin （MOT）, somatostatin （SS） and cholecystokinin （CCK） were determined by radioimmunoassay. Result The plasma MOT level of spleen and stomach deficiency group was significantly higher than that in the normal controls （P 〈0.05）. The plasma level of CCK in liver qi stagnation and spleen qi deficiency group was significantly higher than that in liver qi stagnation group （P 〈0.05）. The plasma level of SS in liver qi stagnation group was significantly higher than that in spleen and stomach deficiency group and liver qi stagnation and spleen qi deficiency group （P 〈0.05）. Conclusion Plasma gastro-intestinal hormone levels in patients with IBS are related to different TCM syndrome type. There is a correlation between the TCM syndrome type of IBS and the gastro-intestinal hormone levels.
