胰头癌外科疗效的临床分析

目的总结胰头癌的外科疗效，探讨提高胰头癌疗效的综合治疗方法。方法回顾性总结、分析172例胰头癌病人的临床资料及外科疗效，并总结同期106例壶腹周围癌（除外胰头癌）的外科疗效。结果29例胰头癌行根治性切除，切除率为16．9％，切除术后1、3、5年存活率分别为61．5%、26．9％、3．8％，中位生存期为14．6个月；未行手术切除者中位生存期为5．7个月。61例壶腹周围癌（除外胰头癌）行根治性切除，切除率为57．5％（61／106），术后5年生存率为26．9％。结论提高胰头癌的手术切除率有望延长病人的生存期，改进、规范胰头癌术式可提高胰头癌的手术切除率和术后长期生存率。

Clinical analysis of surgical treatment of pancreatic head adenocarcinoma

Objective To summarize the efficacy of surgical treatment for pancreatic head ade- nocarcinoma(PHAC)and investigate the comprehensive methods to improve the therapeutic efficacy. Methods The clinical data of 172 patients with PHAC surgically treated in our hospital were retro- spectively analyzed.The efficacy of surgical treatment in 106 cases of periampullary adenocarcinoma (excluding PHAC)was determined.Results The resection rate was 16.9% in the patients(29/ 172).The patients receiving surgical resection had a mean survival time of 14.6 months.For these pa- tients,the 1,3-and 5-year survival rates were 61.5%,26.9% and 3.8%,respectively.Those pa- tients without surgical resection had a mean survival time of 5.7 months.The resection rate of periam- pullary adenoearcinoma was 57.5%(61/106)and the 5-year survival rate in patients with surgical re- section was 26.9%.Conclusions Promotion of the resection rate of PHAC may prolong the survival time of the patients.Radical resection for PHAC rationally increases the resection and long-term sur- vival rates.
