重症急性胰腺炎并发成人呼吸窘迫综合征的治疗体会

目的：讨重症急性胰腺炎并发成人呼吸窘迫综合征（ARDS）的诊断及救治方法。方法：回顾性分析1997年1月至2004年12月收治的49例重症急性胰腺炎患者中并发ARDS23例的临床治疗情况。结果：49例重症急性胰腺炎，发生ARDS者23例（46.9％），死亡6例，病死率26.1％。结论：在重症急性胰腺炎的急性反应期，应反复测定血气分析，可早期诊断ARDS。一旦出现难以纠正的低氧血症，及时行气管插管或气管切开辅助呼吸，可以明显改善缺氧状况，提高重症急性胰腺炎的治愈率。

The Treatment of Adult Respiratory Distress Syndrome in Patients with Severe Acute Pancreatitis

Objective:To explore the diagnosis and treatment of adult respiratory distress syndrome(ARDS) complicated by severe acute pancreatitis(SAP).Methods:Of 49 patients SAP admitted into our hospital from Jan.1997 to Dec.2004,23 with ARDS were retrospectively analysed.Results:There were totally 49 patients with severe acute pancreatitis,and 23 suffered from ARDS(46.9%).Of the 23,six died.The mortality was 26.1%.Conclusion:We must assay blood gas repeatedly during acute response period of SAP.ARDS can be diagnosed early.If low blood oxygen concentration can not be corrected,prompt endotracheal tube or tracheostomy must be done as early as possible.To improve the low blood oxygen concentration is the major procedure in the prevention of the occurrence of ARDS.
