郎格尔汉斯细胞组织细胞增生症的影像学表现

目的　进一步提高对郎格尔汉斯细胞组织细胞增生症 (L angerhans cell histiocytosis,L CH)影像学表现的认识。方法　回顾性分析经实验室及病理证实的 16例 L CH的临床和影像学特点。结果　按传统分型 ,符合勒 -雪氏病 (L S) 3例 ,韩 -薛柯氏病 (HSC) 2例 ,骨嗜酸性肉芽肿 (EGB) 11例。颅骨单发破坏 8例 ,多发破坏 2例 ,下颌骨、肋骨、股骨及尺骨破坏各 1例 ;伴肺内浸润 3例 ,软组织肿块 7例。结论　L CH影像学表现缺乏特异性 ,传统 X线检查仍为首选的影像学检查 ,CT能更准确地显示病变范围。

Radiological Findings of Langerhans Cell Histiocytosis

Objective To enhance our knowledge on radiolog ical findings of Langerhans cell histiocytosis(LCH).Methods The clinical and radiological manifestations of 16 cases of LCH proved by pathology and laboratory were reviewed. Re sults There were three LSs(Letterer-Siwe disease),two HSCs(Hand-Schul ler-Christian d isease)and eleven EGBs(eosinophilic granuloma of bone)in this group. Cranial bon es were involved in ten cases(single lession in 8 and multiple in 2).There was e ach case with osteolysis in mandible, rib, femur and in ulna. The damage of lung were presentet in 3 cases and a soft tissue mass was associated in 7 cases. Conclusion Radiological findings have no characteristic in LCH cas es. Plain radiograph remains the first of choice in imaging modalities. CT may b e useful if plain radiographs inadequately define the extent of the process.
