前列腺癌患者TRUS检查表现结合β2微球蛋白检测与其临床分期及疾病危险度的关系

摘    要:研究前列腺癌患者的经直肠超声检查(TRUS)表现结合β2微球蛋白(β2-MG)检测与其临床分期及疾病危险度的关系。方法 选取海南省东方市东方医院2016年1月至2019年12月诊断的前列腺癌患者120例作为研究对象,其中低危患者62例,中危患者39例,高危患者19例,依据Whitmore-Jewett法对患者进行临床分期,其中A期患者16例,B期患者39例,C期患者44例,D期患者21例。比较不同临床分期和危险度患者的超声特征以及β2-MG的差异,分析患者的疾病分期以及危险度与超声特征及β2-MG的相关性。结果 高危、中危以及低危患者的超声包膜完整情况、结节、前列腺的动脉血流速度(Vmas)、阻力指数(RI)以及β2-MG水平比较,差异均有统计学意义(P均<0.001);A、B、C、D期患者的超声包膜情况、结节、Vmas、RI以及β2-MG水平比较,差异均有统计学意义(P均<0.001);Spearman相关性分析显示,患者的包膜完整性、结节与疾病分期以及危险度呈负相关(P均<0.001),Vmas、RI、β2-MG分别与疾病分期以及危险度呈正相关(P均<0.001)。结论 前列腺癌患者TRUS检查表现结合β2-MG水平检测与其临床分期及疾病危险度呈现显著相关性。

Relationship between TRUS and β 2-microglobulin detection and clinical stage and disease risk in patients with prostate cancer
Abstract:To study the relationship between transrectal ultrasonography(TRUS) examination and β2-microglobulin(β2-MG) detection and clinical stages and disease risk in prostate cancer patients.Methods  A total of 120 prostate cancer patients diagnosed from January 2016 to December 2019 in Dongfang oriental hospital of Hainan province were selected as the research subjects, including 62 low-risk patients, 39 medium-risk patients, and 19 high-risk patients. According to Whitmore-Jewett method, the clinical stages of patients were 16 cases in stage A, 39 in stage B, 44 in stage C and 21 in stage D. Ultrasonic ultrasound was compared with different clinical stages and risk patients. The characteristics and the differences between β2-MG were studied. The relationship between disease stages and risk of patients with ultrasound and β2-MG was studied.Results  There were statistically significant differences between high-risk, medium-risk and low-risk group, nodules, Vmas, RI, β2-MG (P<0.001). Ultrasound capsule status, nodules, Vmas , RI and β2-MG  in stage A, B, C and D patients, the differences were statistically significant (P<0.001). Spearman correlation analysis showed that capsule integrity, nodules were negatively correlated with disease stage and risk (P<0.001). Vmas, RI, β2-MG were positively correlated with disease stage and risk (P<0.001).Conclusions  TRUS and β2-MG were significantly correlated with clinical stage and disease risk in prostate cancer patients.
