肾上腺肿瘤的MRI诊断(附30例报告)

目的:分析肾上腺肿瘤的MRI信号特征与病理类型间的关系。方法:搜集1994年9月~2004年5月经临床手术和病理证实的肾上腺肿瘤30例,均行MRI平扫,其中23例行增强扫描。结果:功能性肾上腺肿瘤13例,其中醛固酮腺瘤5例,皮质醇腺瘤2例,嗜铬细胞瘤6例。无功能性肾上腺肿瘤19例,其中无功能腺瘤11例,神经节瘤1例,皮质腺癌2例,转移瘤3例。结论:MRI是诊断肾上腺肿瘤的较好方法,其化学位移反相位成像技术对鉴别腺瘤、嗜铬细胞瘤与恶性肿瘤具有更大优越性。

The MRI Diagnosis of the Adrenal Tumor

Objective:To analyse comparatively the relationship between the MRI signal characteristics of adrenal tumor and their pathologic types.Methods:From August 1994 to May 2004,30 cases of adrenal tumors confirmed by surgery and pathology were collected.30 cases were performed plain MRI and 23 of them added to enhanced MRI.Results:Functional adrenal tumor was seen in 13 cases,including aldosterone adenoma in 5 cases,cortical adenoma in 2 and pheochromocytoma in 6.Non-functional adrenal tumor was seen in 19 cases,including non-functional adenoma in 11 cases,ganglionic tumor in 1,cortical adenocarcinoma in 2,and metastasis in 3.Conclusion:MRI is a better diagnostic method of the adrenal tumor and its chemistry shift antiphase imaging technique is very useful in differentiating adrenal adenomas,pheochromocytoma and malignant masses.According to the MRI imaging features together with clinic and laboratory results are very helpful to improve the accuracy.
