极低出生体重儿48例临床分析

目的探讨影响极低出生体重儿（VLBWI）存活率的各种因素。方法回顾性分析2006年6月-2008年6月本院新生儿重症监护病房（NICU）收治的48例VLBWI的临床资料。结果治愈35例,治愈率72.9％,死亡8例,病死率16.7％,放弃5例。治愈率随体重（P〈0.01）、胎龄（P〈0.05）的增加而增加。胎龄〈33周41例（85.4％）,45.8％为多胎,39.6％有胎膜早破史,75.0％有窒息史,产科异常是早产的主要原因。窒息、低体温、新生儿呼吸窘迫综合征（NRDS）是常见的并发症。小胎龄、低体重、窒息、NRDS是VLBWI的主要死亡原因。结论加强高危妊娠监测,预防早产是改善VLBWI预后的根本措施,提高新生儿窒息,NRDS的防治水平,注重营养支持是提高VLBWI存活率的关键环节。

Clinical analysis of 48 cases of very low birth weight infant

Objective To study the effects of very low birth weight infants （ VLBWI ） survival of a variety of factors. Methods Retrospective analysis from June 2006 to June 2008 our neonatal intensive care unit （ NICU ） of the 48 cases treated VLBWI clinical data. Results 35 eases cured,the cure rate 72. 9% ,8 eases of death,the death rate of 16.7% , to give up 5. The cure rate with body weight （ P〈0. 01 ） for gestational age （ P〈0. 05 ） increased. Less than 33 weeks gestational age accounted for 41（85.4%） ,45.8 percent for multiple births,39.6 percent have a history of premature rupture of membranes ,75.0 percent have a history of suffocation. Abnormal obstetrics is the main reason for premature birth, asphyxia, hypothermia, newborn respiratory distress syndrome （ NRDS ） is a common complication. Small for gestational age, low body weight, asphyxia, NRDS is VLBWI the leading cause of death. Conclusion Strengthen the conclusions of the high-risk pregnancy monitoring, prevention is prema-ture VLBWI to improve the prognosis of the fundamental measures to improve the neonatal asphyxia, NRDS the prevention and treatment,pay attention to nutritional support is to improve the survival rate VLBWI the key.
