大肠癌肝转移与病理类型的临床观察

目的　探讨大肠癌发生肝转移与临床病理因素的关系。方法　回顾性分析 79例大肠癌肝转移病人的临床特点、血清CEA水平、转移淋巴结、原发灶的病理类型和静脉侵犯。结果　以肝转移灶为首诊原因的有 1 6例。大肠癌肝转移病人远距离淋巴结转移及镜下静脉侵犯发生率升高 ,与对照组比较差异显著 (P <0 .0 5 )。结论　要重视大肠癌肝转移的早期诊断。远距离淋巴结转移或镜下静脉侵袭均预示大肠癌发生肝转移的可能性增加。对可能发生肝转移的高危病人 ,有必要行术中门静脉插管预防性化疗

The clinical observation of relationship between colorectal cancer with hepatic metastasis and the pathologic types

Objective To investigate the relationship between colorectal cancer with hepatic metastasis and its clinical data and its pathologic types.Methods The clinical characteristics,lymph node of metastasis,serum level of CEA,invaded veins and pathologic types of primary lesion were studied in 79 cases of colorectal cancer with hepatic metastasis. Results The hepatic metastatic focus as the earliest manifestation was diagnosed in 21 patients.Both venous invasion and distant metastasis of lymph node in the cases of colorectal cancer with hepatic metastasis were more common than in the control group ( P < 0.05 ).Conclusion Early diagnosis of the hepatic metastasis in colorectal cancer should be highly concerned,and distant metastasis of lymph nodes or venous invasion under light microscopy might indicate the risk of hepatic metastasis from colorectal cancer.The portal venous cannulation chemoprophylaxis might be necessary for the high risk patients with hepatic metastasis.
