早期液体复苏综合疗法治疗感染性休克的临床观察

目的 评价以早期液体复苏为中心的综合疗法治疗感染性休克的临床效果.方法 将282例感染性休克患者分为早期复苏组(n=177)和常规治疗组(n=105),同时根据治疗开始时患者脏器功能受损程度分为轻度、中度和重度,分别比较两组患者治疗前后的APACHE Ⅱ评分以及病死率.结果 轻度脏器功能受损早期复苏组患者治疗后24 h的APACHE Ⅱ评分和病死率明显低于常规治疗组(P<0.01),而在中度和重度水平上两组差异无统计学意义.结论 早期液体复苏能较显著地改善早期感染性休克患者的生存和预后情况.

Clinical analysis of treating septic shock with early fluid resuscitation

Objective To evaluate the effect of complex therapy based on early fluid resuscitation on septic shock.Methods Two hundred and eighty-two patients with septic shock were assigned into early resuscitation group(n=177)and conventional therapy group(n=105).According to the state of organ function at commence of treatment,each group was subdivided into 3 lays: lay A(slight organ dysfunction),lay B(middle organ dysfunction) and lay C(severe organ dysfunction).APACHE II score and case fatality in each group were analyzed.Results APACHE II score and case fatality in lay A patients were significant lower in early resuscitation group than those in conventional therapy group(P<0.01).Meanwhile,there was no significant difference in lay B and lay C patients between two groups.Conclusion In the earlier period of septic shock,early fluid resuscitation could remarkably improve the patients' survival and prognosis.
