颈、右胸、上腹三切口治疗胸上段食管癌的手术探讨

目的 :探讨经颈、右胸、上腹三切口治疗胸上段食管癌手术中应注意的问题。方法 :回顾性总结此类手术患者 80例的手术经验和教训。从术前评估、淋巴结的清扫、喉返神经的保护、胃排空障碍的预防、颈部吻合口的缝合技术、空肠营养管的应用方面探讨了手术中应注意的问题。结果 :术后 1 6例发生并发症 ,77例痊愈出院 ,3例死亡。结论 :通过周密的术前评估和仔细的手术操作可以提高手术疗效并减少术后并发症。

Analysis for surgical treatment of upper thoracic esophageal carcinoma through cervical,right thoracic and abdominal incisions

Objective:To analysis points that should be taken care of during the operation of upper thoracic esophageal carcinoma through cervical, right thorax and upper abdominal incisions. Methods:80 patients were studied retrospectively. The factors that involving preoperative evaluation,lymph nodes dissection, the protection of recurrent laryngeal nerve, the prevention of gastric emptying obstacle and the suturing techniques of cervical stoma were analyzed.Results:The tumor length ranges from 3cm to 10cm,all cases have pathologic diagnosis of squamous carcinoma.There are 16 patients with complications after operation and 3 patients died.Conclusion:Careful preoperative evaluation and fine surgical procedure can improve the operative therapy effects and decrease the rate of postoperative complications.
