老年慢性病患者出院后居家护理干预及其效果评价

目的:探讨居家护理干预对出院后老年慢性病患者的影响。方法:将我院2010年1～12月医院辖区范围、在本院内科出院后、便于随访的老年慢性病患者100例随机分为干预组和对照组。对照组给予一般评估和常规性健康教育,干预组在常规护理方法上给予定期随访及电话咨询,给予家庭护理干预措施。根据心理功能测试(SDS),社会功能评定量表(SDSS)及日常生活能力测试(ADL)等量表监测患者心理功能、社会功能及日常生活能力,评价居家护理干预对老年慢性病患者的影响。结果:干预组老年慢性病患者的心理功能、社会功能及日常生活能力均优于对照组,两组比较差异有统计学意义(P<0.01,P<0.05)。结论:家庭护理干预措施可以明显改善老年慢性病患者的心理功能、社会功能及日常生活能力,提高患者生活质量。

The home care intervention and evaluation of elderly patients with chronic disease after discharge.

Objective：To observe the effect of nursing intervention after discharge of elderly patients with chronic diseases.Methods：From January to December 2010 in the scope of the hospital area,100 cases elderly patients with chronic disease that after discharge and facilitate follow-up were randomly divided into intervention and control groups.Control group was given a general assessment and routine sexual health education,nursing intervention group were given regular follow-up and telephone consultation,to give home care interventions.According to SDS,SDSS and ADL scale monitoring of patients such as psychological function,social function,and daily living skills,evaluation the effect of home care interventions for elderly chronically patients.Results：Intervention group,elderly patients with chronic mental function,social function and daily living skills were better than the control group,the difference was statistically significant（P0.01,P0.05）.Conclusion：Home care interventions can significantly improve elderly patients with chronic mental function,social function and activities of daily living,improve quality of life.
