丙种球蛋白联合地塞米松在小儿特发性血小板减少性紫癜中的疗效观察

目的探讨丙种球蛋白联合地塞米松在小儿特发性血小板减少性紫癜中的疗效。方法选取2009年1月～2011年1月于某院进行治疗的80例小儿特发性血小板减少性紫癜患者为研究对象,将其随机分为对照组(地塞米松组)40例和观察组(丙种球蛋白联合地塞米松组)40例,后将两组患者的治疗总有效率、不良反应发生率及治疗前后的外周血T淋巴细胞亚群、IL-6、TNF-α及IL-18水平进行统计及比较。结果观察组的治疗总有效率高于对照组,CD3+、CD4+、CD4+/CD8+水平高于对照组,CD8+、IL-6、TNF-α及IL-18水平低于对照组,P﹤0.05或P﹤0.01,有显著性或非常显著性差异,但不良反应发生率差异无统计学意义,P﹥0.05。结论丙种球蛋白联合地塞米松在小儿特发性血小板减少性紫癜中的疗效显著,可调节患者的免疫状态,优势明显。

Observation on the effects of gamma globulin combined with dexamethasone in children with idiopathic thrombocytopenic purpura

OBJECTIVE To study the effects of gamma globulin combined with dexamethasone in children with idiopathic thrombocytopenic purpura.METHODS 80 children with idiopathic thrombocytopenic purpura in our hospital from January 2009 to January 2011 were selected as research subjects,and they were randomly divided into control group(dexamethasone group)40 cases and observation group(gamma globulin combined with dexamethasone group)40 cases,then the total effective rate,incidence of adverse reactions and peripheral blood T lymphocyte subsets,serum IL-6,TNF-α and IL-18 before and after the treatment of two groups were analyzed and compared.RESULTS The total effective rate of observation group was higher than that of control group,CD3+,CD4+ and CD4+/CD8+ were higher than those of control group,CD8+,IL-6,TNF-αand IL-18 were lower than those of control group,P﹤0.05 or P﹤0.01,there were significant differences,but incidence of adverse reactions did not have significant differences,P﹥0.05.CONCLUSION The gamma globulin combined with dexamethasone has a significant curative effect in children with idiopathic thrombocytopenic purpura,it can regulate the immune status,and the advantage is obvious.
