下腰背痛并发下肢痛或下腹痛的形态学基础-荧光双标和免疫组化法研究

摘    要:
用逆行荧光素双标法结合免疫组化技术研究了大鼠腰骶部脊神经节(SG)细胞的周围突分支投射及其化学性质.选21只Wistar大鼠,分三组,每组7只.第Ⅰ组,将碘化丙啶(PI)注入坐骨神经,双苯甲亚胺(Bb)注入背部皮肤;第Ⅱ组,将快蓝(FB)注入背肌,核黄(NY)注入坐骨神经;第Ⅲ组,将PI注入背肌,Bb注入膀胱壁.全部动物存活适当时间后,灌注固定,取SG做冰冻切片,用Nikon荧光显微镜观察,结果在三组动物的SG中都发现部分荧光双标细胞.将含有荧光双标细胞的切片,用兔抗CGRP血清做PAP法的免疫组化反应,发现部分荧光双标细胞含CGRP.本研究结果提示发生在SG水平的牵涉痛可能是下腰背痛并发下肢痛或下腹痛的神经基础之一.

Abstract:
Double fluorescent labelling and immunohistochemical methods were used to research the dichotomizing projection of the peripheral process and neuropepdde of rat spinal ganglion(SG) cells in lumbar - sacral region. Twenty -one rats were divided into three groups, seven in each. In the first group, propidium iodide(PI) was injected into the sciatic nerve, and bisbenzimide(Bb) into the dorsal skin. In the second group, fast blue(FB) was injected into the erector spinae muscle, and nuclear yellow (NY) into the sciatic nerve. In the third group, PI was injected into the erector spinae muscle, Bb into the wall of the urinary bladder. After a certain period of survival time, the animals were perfused and fixated. The corresponding SG cells were dissected out, cut on a cryostat, and observed under Nikon fluorescent microscope. The double fluorescent labelled cells were found in SG cells of all the three groups. The sections containing double fluorescent labelled cells were further processed by CGRP - demonstrating PAP immunohistochemical method. The results showed that some of the double labelled cells contained CGRP. These findings suggest that the referred pain occurring on SG level may be one of the causes of low back pain accompanied with pain lower limb or hypogastric organs.
