留置导尿致医源性感染的临床分析及预防对策

目的 探讨预防留置导尿致医源性感染的护理方法.方法 选取2008年1月-2009年1月行留置导尿治疗的80例患者为研究对象,将其随机分为观察组和对照组,对照组患者采用常规的留置导尿护理,观察组患者在对照组基础上应用PDCA循环管理模式进行留置导尿护理管理,将两组患者不同时期的导尿口尿样、接尿口尿样进行病原菌检测,计算有菌率并进行比较,总结留置导尿致医源性感染发生的关键原因及预防措施.结果 经研究比较发现,观察组实施PDCA循环管理模式4个周期后,其尿道口、接口处平均有菌率分别为21.9％、6.9％；对照组患者则为51.5％、22.9％,两组比较,差异有统计学意义(P＜0.05).结论 提高留置导尿的操作水平及规范良好的护理工作,对留置导尿致医源性感染的预防有积极作用.

Clinical analysis and preventive measures for iatrogenic infection caused by indwelling catheter

OBJECTIVE To investigate the care approach to prevent iatrogenic infection caused by indwelling catheter.METHODS A total of 80 patients with indwelling catheter in our hospital for treatment from Jan 2008 to Jan 2009 were selected as research objects.And they were randomly divided into the control group and the observation group.The control group used conventional catheterization care,and the patients in the observation group applied PDCA cycle management model based on catheterization care management in the control group.Pathogens were examined at the catheterization mouth and urine-picking interface at different periods in the two groups of patients,and bacterial counting rate were calculated and compared.The key reasons and preventive measures for iatrogenic infection were summarized.RESULTS The comparative study found that after applying four cycles of the PDCA cycle management model,the average bacterial rate at the urethra and interface were 21.9%,6.9% in the observation group,and 51.45%,22.92% in the control group.There was significant difference(P<0.05).CONCLUSION It has a positive effect to improve the operations and catheterization standard for prevention of indwelling catheter-induced iatrogenic infection.
