剪切波定量超声弹性成像技术在慢性肾病中应用的初步研究

摘    要:目的 比较慢性肾病与正常肾脏实质部的弹性差异,以探讨剪切波定量超声弹性成像技术在肾脏疾病的应用价值.方法 采用剪切波弹性成像技术分别测量60名健康体检者与57例慢性肾病(CKD)患者肾脏实质部杨氏模量值并进行比较.结果 CKD患者肾实质部杨氏模量值为(6.363±2.944)kPa,健康体检者杨氏模量值为(3.671±1.861)kPa,两者比较差异有统计学意义(t=8.402,P=0.000).CKD患弹性模量值在5.0~6.0 kPa所占百分比较多,约为26%(15/57),而健康体检者弹性模量值在1.5~2.0 kPa所占百分比较多,约为18%(11/60).结论 CKD患者肾实质部杨氏模量值大于健康人肾实质部杨氏模量值.剪切波超声弹性成像可能为肾脏疾病提供常规超声之外的诊断信息.

Abstract:Objective To investigate the renal parenchyma with quantitative shear-wave ultrasound elasticity technique,in order to observe the value of elastography in chronic kidney disease.Methods Shear-wave ultrasound elasticity imaging were performed in 57 chronic kidney disease patients and 60 healthy people.The quantitative elasticity value of renal parenchyma were measured and compared.Results The elasticity value of renal parenchyma in chronic kidney disease was(6.363±2.944)kPa,26%(15/57)of which was in the 5.0-6.0 kPa,whereas of renal parenchyma in control group was(3.671±1.861)kPa,18%(11/60)of which was in the 1.5-2.0 kPa,which statistically significant difference(t=8.402,P=0.000).Conclusions The quantitative elasticity value of renal parenchyma in chronic kidney disease was higher than that in control group.Shear-wave ultrasound elasticity imaging could provide additional information in the diagnosis of chronic kidney disease.
