2007～2009年我院金黄色葡萄球菌的耐药性变迁

目的：对我院三年来金黄色葡萄球菌的耐药性变迁进行分析,并对耐甲氧西林金黄色葡萄球菌（MRSA）与甲氧西林敏感金黄色葡萄球菌（MSSA）的耐药性差异做对比,为临床合理使用抗生素提供依据。方法：采用VITEK-2全自动微生物分析仪和WHONET软件对570株金黄色葡萄球菌的耐药情况进行回顾性分析。结果：金黄色葡萄球菌对利奈唑胺、万古霉素和替考拉宁具有很高的敏感性;对头孢类素类、苯唑青霉素、红霉素和氨苄西林/舒巴坦、环丙沙星的耐药率有上升的趋势;MRSA对苯唑西林、头孢西丁、庆大霉素、环丙沙星、左旋氧氟沙星、复方新诺明、克林霉素、红霉素和米诺环素的耐药率都明显高于MSSA的耐药率,两者间有非常显著性差异（P〈0.01）。结论：金黄色葡萄球菌的耐药性逐渐升高,特别是对MRSA应引起临床的重视,要依据实验室的药敏结果,优化选择抗生素,才能更有效地控制感染。

The change in the antibiotic resistance of staphylococcus aureus in our hospital from 2007 to 2009

Objective：To analyze the change in the antibiotic resistance of staphylococcus aureus during the three years in our hospital,and to compare the difference in the resistance of MRSA and MSSA to various drugs,so as to provide evi dence for reasonable clinical treatment with antibiotics.Methods：The results were identified by using VITEK-2 analyzer and WHONET in a retrospective analysis of the antibiotic resistance in 570 strains of staphylococcus aureus.Results：Staphylococcus aureus was highly sensitive to Linezolid,Vancom ycin and Teicoplanin;there was a rising tendency in its resistance rate to Cephalosporins,Oxacillin,Erythromycin,Ampicillin-salbactamand and ciprofloxacin;the resistance rate of MRSA to Oxacillin,Cefoxitin,Gentamycin,Ciprofloxacin,Levofloxacin,Cotrimoxazole,Clindamycin,Erythromycin and Minocycline was obviously higher than that of MSSA,which could be shown in an significant difference between them（P0.01）.Conclusion：There should be timely monitoring of staphylococcus aureus concerning its resistance,and special attention should be paid to MRSA in order to avoid in-hospital infection.
