介入封堵术治疗伴有卵圆孔未闭偏头痛患者的效果观察

目的评价介入封堵术治疗伴有卵圆孔未闭偏头痛患者的效果。方法选取2013年1月～2014年1月在本院神经内科就诊的偏头痛患者，同时接受经颅多普勒和经胸心脏超声（TrE）发泡实验阳性，后经食道超声确诊为卵圆孔未闭，并在本院心内一科接受卵圆孔未闭封堵治疗的患者30例。术前及术后行HIT-6评分对头痛症状是否缓解进行评估，术后行，TTE发泡实验复查封堵效果。结果30例患者手术均成功，在院及随访期间无手术并发症出现。术前的HIT-6评分为（70±5）分，术后的HIT-6评分为（49±3）分，差异有统计学意义（P〈0．05）。术后1个月复查TTE均显示封堵器位置良好。TTE发泡实验27例分流消失，3例仍有残余分流。结论卵圆孔未闭封堵术可以明显缓解伴有卵圆孔未闭的偏头痛患者的头痛症状。

Effect observation of interventional closure in the treatment of patients with migraine combined with patent foramen ovale

Objective To evaluate the effect of interventional closure in the treatment of patients with migraine combined with patent foramen ovale. Methods 30 patients with migraine who were tested positive in contrast transcranial Doppler and echocardiographic contrast and defined patent foramen ovale by transoesopheageal echocardiography（TEE） at the department of neurology in our hospital from January 2013 to January 2014.Procedures of transcatheter closure of patent foramen ovale were performed for all patients at department of cardiology in our hospital.After transcatheterclosure the patients were re-examined by TCDs and rIFE and interviewed with headache impact test-6 （HIT-6）questionnaire to elevate improvement of migraine.The data were analyzed. Results Percutaneous closure was successful in all patients.In the court and during the follow-up period had no complication appeared.Preoperative HIT-6 score was （70＋ 5）points,postoperative HIT-6 score was（49＋3） points,with statistical difference（P〈0.05）.Mter operation 1 month,re-examined by TTE for evaluate the site of occluder and re-examined rITE contrast,the shunt was disappear in 27 patients, 3 cases were still residual shunt. Conclusion Interventional closure can obviously relieve headache of patients with migraine combined with patent foramen ovale.
