婴幼儿肺炎心衰患者内源性洋地黄样因子的变化

摘    要:测定了67例正常儿、31例婴幼儿肺炎和30例婴幼儿肺炎心衰的血清内源性洋地黄样因子(EDF)动态变化。结果表明:肺炎组急性期EDF水平显著高于正常组(P<<0.001),恢复期复查降至正常水平。肺炎心衰组EDF水平显著高于正常组(P<<0.001)及肺炎组(P<0.001),恢复期也降到正常水平。结果提示,婴幼儿肺炎时动态监测EDF水平变化,有助于及时诊断心衰,可作为心衰的一个实验室指标。

Abstract:Bronchopneumonia complicated with congestive heart failure is the main cause of death in infant bronchopneumonia. The dynamic change of endogenous digitalis-like factor(EDF) was measured on 67 normal infants,31 infants with bronchopneumonia complicated with heart failure to find a concise laboratory indication.The results showed:1.the EDF level in bronchopneumonia in the acute period was significantly higher than that in the normal group (P<0.001) and returned to the normal level in the recovery period;2.the EDF level in bronchopneumonia complicated with heart failure was much higher than that in the normal group (P<0.001) and the group of bronchopneumonia (P<0.001)and also returned to the normal level in the recovery period.The EDF level in bronchopneumonia rose to overcome the high pressure in pulmonary artery and rose remarkably in bronchopneumonia complicated with heart failure. Even then the risen EDF level could not meet the body demand and finally heart failure took place. The relative or absolute insufficient EDF secretion may be the mechanism of heart failure and also the theoretical basis of digitalis for treating heart failure. Measuring the EDF level continually is helpful to diagnose heart failure in time. Further studies are needed to resolve the problems about the EDF in infant bronchopneumonia.
