稳定期慢性阻塞性肺疾病患者的营养干预

摘    要:目的 通过营养干预改善稳定期慢性阻塞性肺疾病患者的营养状况和肺通气功能。方法 将88例研究对象随机分为干预组(46例)和对照组(42例),对干预组患者,参照膳食调查结果予以针对性的饮食指导;比较各组干预前后以及干预组和对照组之间各项研究指标的变化。结果干预组患者体重、上臂肌围、转铁蛋白、白蛋白和用力肺活量得以维持(P>0.05);上臂肌围和前白蛋白明显提高(P<0.01);肱三头肌皮褶厚度和第1秒用力呼气量虽略有减低(P<0.05),但较对照组减低幅度明显小,而对照组患者所有指标均减低(P<0.05)。结论 对稳定期慢性阻塞性肺疾病患者予以针对性的饮食指导,不仅改善患者的营养状况,而且有利于肺通气功能的保护。

Abstract:Objective To improve their nutritional status and ventilation function in patients with chronic obstructive pulmonary disease in the stable phase by nutritional intervention. Methods Totally, 88 subjects were randomly divided into two groups, one with nutritional intervention (46 cases) , including relevant diet advice to patients in light of the data from a dietary survey and the another one control group (42 cases). Changes in study indices were compared between the two groups. Results Indices of weight, upper arm circumference, serum transferrin (TF) , albumin (Alb) , and forced vital capacity (FVC) were kept stable (P>0. 05) , mid-upper arm circumference and serum prealbumin (PA) were clearly improved (P<0. 01) , and thickness of skinnfold (TSF) of triceps muscle of arm and forced expiratiry volume at the first second (FEV1) slightly decreased (P <0. 05) in patients with nutritional intervention. Although, TSF and FEV1 decreased a little in nutritional intervention group, but significantly less than those in the control group. All indices decreased in the control group (P < 0. 05). Conclusions Diet advice to the patients with COPD and cor pulmonale in the stable phase could not only improve their nutritional status, but also be helpful to protect their ventilation function.
