摘    要:目的研究血清降钙素原(PCT)与内毒素在肾移植术后并发肺部感染患者诊断中的实用价值。方法回顾性研究肾移植术后并发肺部感染患者102例,根据肺部感染的病原体诊断标准分为细菌组(32例)和非细菌组(70例),检测并比较两组PCT和内毒素值,将两组数据进行统计学比较。结果肾移植术后并发肺部细菌感染患者血清中PCT增高与非细菌组比较差异有统计学意义(P〈0.05)。内毒素增高与非细菌组比较差异有统计学意义(P〈0.05)。PCT对肾移植术后合并细菌性肺部感染患者诊断敏感度为90.6%,特异度为94.3%;内毒素对肾移植术后合并细菌性肺部感染患者诊断敏感度为65.6%,特异度为64.3%。细菌组患者血清中PCT增高例数和内毒素增高例数差异有统计学意义。结论监测PCT值对肾移植术后是否并发肺部细菌感染患者具有明显的临床诊断价值。

Application of procalcitonin and endotoxin in the diagnosis of pneumonia patients after kidney transplantation

Abstract:Objective To evaluate the practical value of procalcitonin (PCT) and endotoxin in diagnosis of pneumonia patients after kidney transplantation. Methods 102 cases of pneumonia patients after kidney transplantation were reviewed. According to the diagnostic criteria of pathogens of pneumonia, 32 patients were enrolled in bacterial group and 70 patients were enrolled in non-bacterial group. The data of the PCT level and endotoxin level of the two groups were determined and compared with each other. Results There was significant difference (P 〈 0.05) between the PCT level of bacterial pneumonia patients after kidney transplantation and non-bacterial group. The difference between endotoxin level of bacterial pneumonia patients after kidney transplantation and non-bacterial group was significant (P 〈0. 05 ) . The sensitivity and specificity of PCT were 90.6% and 94. 3% respectively in bacterial pneumonia patients after kidney transplantation. The sensitivity and specificity of endotoxin were 65.6% and 64. 3% respectively in bacterial pneumonia patients after kidney transplantation. The PCT level and endotoxin level of patients of bacterial group was significant (P 〈 0. 05) . Conclusion Monitoring the PCT level in bacterial pneumonia patients after kidney transplantation would have an obvious clinical significance in diagnostic value.
