国产促性腺激素释放激素(戈那瑞林)的临床应用

摘    要:目的:探讨国产促性腺激素释放激素(GnRH)——戈那瑞林代替人绒毛膜促性腺激素(hCG)诱发排卵的效果。方法;对采用克罗米芬(CC)及人绝经期促性腺激素(hMG)促排卵的104个周期,于卵泡成熟时,给予戈那瑞林,并与hCG诱发排卵的103个周期进行对照,观察排卵率、妊娠率及黄体功能。结果:用戈那瑞林诱发排卵,排卵率分别为90%(CC促卵泡发育)与91.66%(hMG促卵泡发育);妊娠率分别为18.51%(CC促卵泡发育)与19.44%(hMG促卵泡发育)。与 hCG组相比,差异无显著意义。黄体期 E_2、P及卵巢血供情况较 hCG组明显降低。结论:以戈那瑞林代替hCG具有相似的排卵率与妊娠率,但部分患者黄体功能不足,需早期进行黄体支持疗法。

Abstract:Objective:To evaluate the efficacy of Gonadorelin instead of hCG for triggering ovulation. Methods:Go-nadorelin was given for triggering follicular final maturation in 104 clomiphene citrat and human menopausal go-nadotropin stimulating cycles. The rates of ovulation and pregnancy and the luteal gonadotropin stimulating cycles. The rates of ovulation and pregnancy and the luteal function were analyzed and compared with that of hCG. Results: The rates of ovulation were 90 and 91.66 per 100 cycles. The rates of pregnancy were 18.52 and 19.44 per 100 cy-cles. No differences were observed in ovulation rate and pregnancy rate between the groups of Gonadorelin and hCG. The average level of estradiol and progesterone and the ovarian circulation in the luteal phase decreased significantly than that of the hCG groups. Conclusion:The use of Gonadorelin instead of hCG for triggering ovulation is able to successfully induce ovulation and pregnancy. But a part of the patients were inadequate luteal function. It is necessary for them to receive the treatment with luteal support in the early luteal phase.
