PTEN蛋白在肝细胞癌中的表达及其意义

摘    要:目的 通过检测肿瘤抑制基因产物PTEN蛋白在肝癌中的表达情况,探讨其作为一种评估肝细胞癌(HCC)生物学特征的分子标记的意义。方法 用免疫组化技术检测3组共43例(孤立性大肝癌、小肝癌及多结节肝癌组)HCC病例与6例正常肝组织中PTEN蛋白的表达。分析HCC中的PTEN蛋白表达与各临床病理特征的关系。结果 PTEN蛋白在正常肝组织及HCC癌旁组织中表达水平高于癌组织,差异具有显著性(P<0.05),但正常肝组织与癌旁组织间表达无显著性差异(P>0.05)。分化程度高的HCC中PTEN表达高于低分化者,无静脉浸润者高于有静脉浸润者,Child A级者高于B级和C级,差异均具有显著性(P<0.05)。PTEN蛋白在大肝癌组表达高于多结节组,但大、小肝癌组间表达无显著性差异(P>0.05)。结论 PTEN蛋白可作为检测肝癌生物学特征的一种分子标记。PTEN检测结果支持孤立性大肝癌较多结节肝癌具有更好的预后。

Abstract:Objective To detect the expression of PTEN protein in hepatocellular carcinoma (HCC) to determine whether it can be used as a new molecular label for evaluation of biological characteristics of HCC. Methods The expression of PTEN protein was immunohistochemically determined in histological specimens from 43 patients with HCC and 5 controls. Meanwhile, the relation of PTEN expression to clinical pathological characteristics of HCC was explored. Results PTEN expression was significantly stronger in para-neoplastic liver tissue than in the tumor tissue (P<0. 05). There was marked difference in the expression between HCC with various characteristics of Edmond-son stages, vein invasion or preoperative Child classification (P<0. 05). HCC with higher differentiation, no vain invasion and grade A showed remarkably stronger PTEN expression (P<0. 05). The solitary large HCC was stronger in PTEN expression than the multinodular one. No significant difference in the expression was found between the small HCC and solitary large HCC. Conclusions PTEN expression might be used a molecular label to determine biological characteristics of HCC. The results of PTEN expression determination indicates that solitary large HCC might show better prognosis than multinodular HCC.
