带锁钉治疗胫骨骨折(附36例报告)

目的 :观察带锁髓内钉治疗胫骨骨折的疗效。方法 :1997年 7月～ 1998年 10月 ,对 3 6例胫骨骨折采用带锁髓内钉固定。其中男 2 1例 ,女 15例 ,年龄 18～ 62岁。闭合性骨折 19例 ,开放性骨折 17例。手术采用扩髓 14例 ,动力型固定 4例 ,静力型变为动力型固定 15例。结果 :3 6例随访 ,平均 18个月 ,骨折全部愈合 ,平均愈合时间 11周 ,最终结果评分根据Tohner-Wruh评分标准 ,优 3 2例 ,良 4例。结论 :带锁髓内钉治疗胫骨骨折是一种较好的内固定方法。具有创伤小 ,固定坚强 ,应力遮档小 ,骨折愈合率高和感染率低等优点。

Fractures of the Tibia Treated with a Interlocking Intramedullary Nail

Objective: To observe effect of treating tibial fracture by interlocking intramedullary nail. Methods: From July 1997 to October 1998, 36 patients with tibial fractures were treated with the interlocking nail. Of the patients, 21 were male and 15 female, aged ranging from 18 to 62 years old. Close fracture were 19 cases and open fracture 17 cases. Inserting the nail with reamer were in 14 cases, and dynamic interlocking were 4 cases. Dynamic interlocking replaced static one in 15 cases. Results: 36 cases were followed-up for an average 18 months. All fractures were completly healed. The time for fracture union averaged 11 weeks and evaluated according to the criteria of Johner and Wruh. There were 32 assessed as excellent, 4 as good. Conclusion: interlocking intramedullary nail is a good internal fixation for treatment of fractures of the tibia. It has the advantages of less trauma, firm fixation, less stress-shielding effect, high rate of fracture healing and less chance of infection.
