急性心肌梗塞溶栓疗法在基层医院的临床应用

目的 :探讨基层医院开展急性心肌梗塞 (AMI)静脉溶栓疗法的应用前景。方法 :对本院近 4年入院的AMI患者 1 2 5例 ,选择符合溶栓条件、发病≤ 1 2h的AMI患者 70例 ,应用尿激酶进行静脉溶栓治疗 ,总结冠脉再通、不良反应情况 ,并回顾性选择发病≤ 1 2h的AMI患者 6 4例为对照组 ,对两组入院 4周的心脏事件发生率、死亡率进行对比分析。结果 :溶栓治疗组的冠脉再通率、不良反应与国内报道相似 ,而病死率、心脏事件发生率与对照组比较明显降低 ,有显著性差异 (P <0 .0 1 ) ,能进行溶栓病例占同期住院AMI患者 5 6 .0 %。结论 :基层医院开展AMI静脉溶栓疗法 ,疗效确切、不良反应少 ;能溶栓病例与国内报道比较 ,入选率更高

Clinical application in the skeleton hospitals of thrombolytic therapy of acute myocardial infarction

Objective: To explore the applied foreground of vena thrombolytic therapy of acute myocardial infarction (AMI) in the skeleton hospitals. Methods: From the 125 AMI patients who were admitted into our hospital in the recent 4 years, we chose 70 cases who accorded with the conditions and had been ill for no less than 12 hours, gave them thrombolytic therapy by using urokinase, summarized the cases of vena clearing again and side effects, compared with another 64 AMI patients who had been ill for no less than 12 hours, and analyzed the attack rate and death rate of the two groups during their being in hospital by comparison. Results: The rate of vena clearing again and side effects by thrombolytic therapy are similar to those findings reported in our country, but the attack rate and death rate of the tested compared are reduced and different obviously compared with the contrasted group, and 56.0 percent of the AMI patients can be treated by thrombolytic therapy. Conclusion: With the development of vena thrombolytic therapy of AMI in the skeleton hospitals, the therapy is effective, and has few side effects. Compared with the report in our country, the selected rate of the cases who can be cured by thrombolytic therapy is higher.
