肿瘤标志物CA19-9在消化道肿瘤诊断中的应用

目的探讨CA19-9测定在消化道肿瘤诊断中的意义。方法采用化学发光法,对40例正常对照以及经明确诊断的220例消化道恶性肿瘤和良性疾病患者进行血清CA19-9检测。结果胰腺癌患者血清CA19-9含量和阳性率为215.27±147.47U.ml-1和87.9%,肝癌为201.5±165 U.ml-1和60%,胃癌为125±85 U.ml-1和45%,肠癌为247.5±217.5 U.ml-1和61%,慢性肝炎为14.9±9.9 U.ml-1,慢性胃炎为16.1±9.3 U.ml-1,正常人为14.2±8.3 U.ml-1。恶性肿瘤组血清CA19-9分别与正常组相比,差异有统计学意义（P〈0.01）,良性疾病组血清CA19-9分别与正常组相比无统计学差异（P〉0.05）,恶性肿瘤组血清CA19-9分别与良性疾病组相比,差异有统计学意义（P〈0.01）。结论血清CA19-9是诊断胰腺癌、肝癌、胃癌和肠癌的一项特异性较高、敏感度较强的肿瘤标志物,可作为消化道肿瘤诊断的临床参考指标。

Diagnostic significance of CA19-9 in digestive system cancer

Aim To explore the significance of CA19-9 in the diagrosis of digestive system cancer.Methods 40 normal controls as well as 220 cases with a clear diagnosis of gastrointestinal cancer and benign disease patients weretested on serum CA19-9 by cbemiluminescere.Results CA19-9 in the serum of patients with pancreatic cancer was 215.27 ± 147.47 U·ml-1 and the positive rate was 87.9%,liver cancer 201.5±165 U·ml-1 and 60%,gastric cancer 125±85 U·ml-1 and 45%,colorectal cancer 247.5±217.5 U·ml-1 and 61%.CA19-9 in the serum of patiento with chronic hepatitis was 14.9±9.9 U·ml-1,chronic gastritis 16.1±9.3 U·ml-1,and normal 14.2±8.3 U·ml-1.There is a significant difference between malignant disease group and the normal group（P〈0.01）,while there is no difference between benign disease group and the normal group（P〉0.05）.There is a significant difference between malignant disease group and benign disease group（P〈0.01）.Conclusion CA19-9 is a highly specifice sensitive tumor marker for the diagnosis of pancreatic cancer,liver cancer,stomach cancer and colorectal cancer.It can be used as an indicator for the diagnosis of gastrointestinal cancer.
