变态反应性支气管肺曲菌病的影像学表现

目的:探讨变态反应性支气管肺曲菌病的CT及X线胸片的影像学特点。方法:回顾性分析8例变态反应性支气管肺曲菌病患者的CT及胸资料,所有患者均依照目前通用的ABPA诊断标准做出诊断。结果:8例ABPA患者的胸片中7例发作期见游走性浸润影,3例有局限性肺不张,7例见支气管扩张,5例合并牙膏样、树枝样或指套样阴影。CT显示8例患者均有囊状支气管扩张并支气管黏液栓形成及以树芽征为主要表现的小叶中心结节形成。3例患者有段或叶的实变、不张。结论:变态反应性支气管肺曲菌病的CT及胸片表现具有相对特征性。

Radiological Study of Allergic Bronchopulmonary Aspergillosis

Objective:To evaluate the radiological findings of allergic bronchopulmonary aspergillosis(ABPA)in 8 patients in order to improve diagnostic efficacy.Methods:The CT scan and X-ray films of chest of 8 patients with ABPA were analyzed retrospectively,all patients were diagnosed according to the standard diagnostic criterion.Results:Radiographs showed patchy infiltration in 7 cases,atelectasis in 3 cases,bronchiectasis in 7 cases,and thick,branching structures resembling toothpaste,fingerstalls or branches in 5 cases.CT showed bronchiectasis,bronchial mucoid impaction,and tree buds signs in all patients;Segmental or lobar consolidation or atelectasis were observed in 3 cases.Conclusion:ABPA has characteristic findings in radiological examinations.It is an optimal approach to exploit plain radiography combined with CT in diagnosis and treatment evaluation of ABPA.
