经骶尾后路低位直肠肿瘤切除术的临床应用

目的:比较经骶尾后路低位直肠肿瘤切除术式与Miles手术的临床应用价值.方法:46例直肠癌分为2组,26例行经骶尾后路保留肛门肿瘤切除术(A组),20例接受Miles手术治疗(B组),总结临床治疗效果.结果:46例手术顺利,无手术死亡,3年生存率100%,无局部复发.A组平均手术时间1h 40min,并发症发生率为11.5%,术后肛门排便功能恢复良好.B组平均手术时间2h 30 min,并发症发生率10.0%,术后失去自主肛门排便功能.结论:经骶尾后路低位直肠肿瘤切除术是一种术式简捷、疗效肯定、真正意义上保留肛门排便功能的手术方式,对早期低位直肠癌肿,可以达到根治目的.

Posterior resection via sacrum and coccyn in the treatment of low rectal cancer

Objective:To evaluate the value of posterior resection in the treatment of low rectal cancer and compare it with Miles operation.Methods:A total of 46 patients were included in this study,26 cases undergoing sphincter-saving proctectomy were treated by posterior resection(A group),the other 20 cases were treated by Miles operation(B group).The clinical results were evaluated.Results:All the operations were performed successfully,no operative death.Follow-up for 3 years,survival rate was 100% and no one had local recurrence.In group A,the mean operation time was 1 hour and 40 minutes,the complication rate was 11.5%,and the sphincter function resumed after operation.In group B,the mean operation time was 2 hour and 30 minutes,the complication rate was 10.0%,and the sphincter function lost after operation.Conclusion:Posterior resection via sacrum and coccyx was simple and effective in the treatment of low rectal cancer.It can save sphincter function and play radical resection role in the treatment of low rectal cancer.
