不稳定骨盆骨折的手术治疗

目的评价切开复位内固定治疗不稳定性骨盆骨折的安全性及疗效。方法2003年6月至2008年3月38例骨盆骨折按Tile分类确定为不稳定性骨盆骨折,B1型5例,B2型7例,B3型4例,C1型8例,C2型9例,C3型5例。均行切开复位内固定,其中B型骨折应用骨盆重建钢板固定,C型骨折应用骨盆重建钢板、骶骨棒和骶髂关节螺钉固定。结果38例患者均得到随访,随访时间9-29个月,平均15.4个月。骨折愈合,骨盆环无畸形,下地行走,无腰腿痛及步态异常;功能评分：优占83.4%;良占8.9%,可6.7%,优良率达到占93.3%。结论切开复位内固定可重建骨盆的稳定性和早期活动,减少术后并发症发生率,是治疗不稳定骨盆骨折的有效方法。

The surgical treatment of unstable pelvic fracture

Objective To evaluate results of open reduction and internal fixation to unstable pelvic fractures. Methods From June 2003 to March 2008,38 cases had been treated by open reduction and internal fixation. According to Tile classification, 38 patients of unstable pelvic fracture were classified, 5 cases were Tile B1, 7 cases were TileB2, 4 cases were Tile B3, 8 cases were Tile C1, 9 cases were Tile C2, 5 cases were Tile C3. Pelvic reconstruction plat was performed in Tile B, sacrum cudgel,pelvic reconstruction plat and iliosacral screw were performed in Tile C. Results 38 cases were followed up 9to29 months, the average was 15 months. The results showed that all cases with fractures got healed.The pelvic abnormity was rectified without pain and gait disturbance. The rates of excellent and good were 93.3%. Conclusion The treatment results are satisfied of unstable pelvic fractures by open reduction and internal fixation operation, It has good stability , early movement and decrease of perioperative complications and offers a new method for treatment of severely unstable pelvic fractures.
