摘    要:
目的比较光学低相干反射仪LENSTAR和光学相干生物测量仪IOLMaster测算人工晶状体(IOL)度数的差异,分析两者的精确性和一致性。方法 72例(122眼)白内障患者术前分别用LENSTAR和IOLMaster两种光学生物测量仪测量眼轴长度(AL)、角膜屈光度(K1和K2)和前房深度(ACD),并分别应用SRKⅡ、SRK/T、HofferQ、Holladay和Haigis公式计算拟植入AcrySofSA60ATIOL的度数,术后目标屈光度为正视。结果两种测量仪所得参数:AL差异为(0.02±0.10)mm,ACD差异为(-0.02±0.17)mm,差异均无统计学意义(P>0.05);K1、K2和(K1+K2)/2差异分别为(-0.05±0.21)D、(-0.12±0.20)D和(-0.08±0.14)D,差异有统计学意义(P<0.05)。两种测量仪测量的参数AL、K1、K2和ACD的Pearsonr值分别是0.999、0.991、0.992和0.927,具有良好的相关性。应用5种IOL度数计算公式得出的IOL度数在两种测量仪之间,差异无统计学意义,有良好的一致性。结论 LENSTAR和IOLMaster测量仪在测量结果和计算IOL度数方面有良好的相关性和一致性,均可用于白内障手术前精确的IOL度数测算。

Abstract:
Objective To compare the accuracy and consistency in ocular biometric measurements and intraocular lens (IOL) power calculation between the new optical low-coherence reflectometry LENSTAR and partial coherence interferometry IOL Master. Methods The clinical data of 122 eyes from 72 cataract patients were analyzed. All patients were measured with LENSTAR and IOL Master. Repeated measurements of axial length (AL), corneal refractive power (K1 and K2), and anterior chamber depth (ACD) were performed by the same examiner with both devices. Power cal- culations for AcrySof SA60AT IOL were compared between the two devices by using five different formulas (SRKⅡ, SRK/T, Hoffer Q, Holladay and Haigis). The target was emmetropia. Results Among the biometry parameters, there was no significant difference in AL and ACD between the two devices (P 〉 0.05). K1 (-0.05±0.21) D, K2 (-0.12±0.20) D and [ K1 + K2 ]/2 (-0.08±0. 14) D values were significantly different between the two devices (P 〈 0.05 ). The correlations of AL, K1, K2, and ACD between the two devices were high, as the values of Pearson r for AL, K1, K2, and ACD were 0. 999, 0. 991,0.992 and 0. 927 respectively. The mean differences in IOL power calculations based on the five formulas between the two devices were not significant. Conclusions New optical low-coherence reflectometry provides measurements that correlate well to those of partial coherence interferometry, thus it was a precise device that could be used for the preoperative examination of cataract patients.
