针刺左侧少海穴对健康青年大脑中动脉血流动力学影响的研究

摘    要:目的:为临床应用针刺少海穴提供大脑中动脉(MCA)血流动力学变化的参考数据。方法:选20名健康青年,针刺左侧少海穴,采用经颅多普勒超声仪分别检测并记录进针前10 min、进针时、行针时、出针时及出针后10 min的双侧MCA的收缩期最大血流速度(Vs)、舒张期最大血流速度(Vd)、平均血流速度(Vm)、搏动指数(PI)及阻抗指数(RI),所得数据应用SPSS 17.0统计软件处理分析。结果:针刺左侧少海穴行针时双侧MCA的Vm参数均较进针前10min下降(P<0.05);右侧MCA的Vs行针时参数较进针前10 min下降(P<0.05);左侧MCA的Vs进针时、行针时、出针时参数分别较进针前10 min下降(P<0.05)。结论:针刺左侧少海穴可降低双侧MCA血流速度,对双侧MCA血流动力学有负性影响,临床治疗脑缺血性疾病慎用少海穴。

Abstract:Objective: To provide a reference for the clinical use of Shaohai acupoint( HT3) and study its effects on the hemodynamics of MCA. Methods: 20 healthy young adults were selected and given acupuncture at left HT3. The transcranial doppler ultasound device was used to measure and record the bilateral MCA peak velocity of systole( Vs),peak velocity of diastole( Vd), mean velocity( Vm),pulsatility index( PI),and resistance index( RI) values at 10 min before needling,during needle insertion, after needle manipulation,during needle removal,and 10 min after needle removal. SPSS 17. 0 software was used to do the statistical analysis on the data. Results: The bilateral MCA Vm values of needling left HT3 were lower at needling manipulation than that at 10 min before needle insertion( P < 0. 05). Right side MCA Vs values during needle manipulation were lower than those at 10 min before needling( P < 0. 05). Left side MCA Vs values at needle insertion,needle manipulation,and needle removal were all lower than those at 10 min before needling( P < 0. 05). Conclusion: Needling left Shaohai acupoint can lower the blood flow velocity in bilateral MCA. It has a negative effect on the hemodynamics of bilateral MCA,and therefore when treating cerebral ischemic diseases,HT3 should be used with caution.
