宫颈癌及上皮内瘤变中p16表达与预后关系的研究

目的研究宫颈癌及宫颈上皮内瘤变中p16基因表达的，临床意义。方法收集我院1996—2006年间妇科门诊活检及住院手术标本共164例，其中宫颈鳞状细胞癌86例，有淋巴结转移者22例，无淋巴结转移者28例，宫颈上皮内瘤变（CIN）78例，复习常规病理HE切片，并采用S-P免疫组化法对86例宫颈鳞癌和78例宫颈上皮内瘤变（CIN）蜡块标苓进行标记，观察P16基因的表达情况并进行对照分析。结果P16基因在宫颈鳞癌及CINⅠ、CINⅡ、CINⅢ中的阳性表达率分别为3g．3％（33／86）、70．0％（14／20）、484％（16／33）、44．0％（11／25），明显低于正常宫颈组织93.3％（28／30）（P〈0．05）；有淋巴结转移者和无淋巴结转移者P16基因阳性表达率分别为27．2％（6／22）、535％（15／28）（P〈005）。结论P16基因表达与宫颈鳞癌、CIN病变程度及淋巴结转移有关。

The Clinical Significance of Changes Expression of p16 in Cervical Carcinoma and Cervical Intraepithelial Neoplasm

Objective To study the clinical significance of changes of expression of p16 in cervical carcinoma and cervical intraepithelial neoplasia. Methods 164 cases of samples are from clinic and in-patient gynecologic department of our hospital（1996-2006）, inclnding 86 cases of cervical carcinoma（adenoid-ambulant, n=22; adenoid-unambulant,n=28） and 78 cases of cervical intraepithelial neoplasia（degree Ⅰ ,n=20; degree Ⅱ, n=33 ; degree Ⅲ, n=25）. The ratios ofpositive expression of the p16 in 164 cases of cervical carcinoma and cervical intraepithelial neoplasia were observed by S-P immune-histochemistry method. Results The levels of rations of positive expression of the p16 in the samples of cervical carcinoma and cervical intraepithclia neoplasia were lower than that of normal samples（P〈0.05）; The levels of ratios of positive expression of the p16 in the samples of cervical carcinoma with adenoid-ambulant were lower than that of the cervical carcinoma without adenoid-ambulant（P〈0.05）. Conclusion The expression of the p16 were related to the cervical carcinoma and its ambulant and the degree of cervical intraeplthelial neoplasia.
