氨氯地平对脑梗死后遗症期的高血压患者颈内动脉血流动力学的影响

摘    要:目的 研究氨氯地平对脑梗死后遗症期的高血压患者颈内动脉血流动力学的影响.方法 80例脑梗死后遗症期的高血压患者随机分为治疗组和对照组,每组40例.治疗组服用氨氯地平作降血压治疗,对照组不服药.采用彩色多普勒超声仪测定治疗组和对照组颈内动脉的血流动力学指标.结果 治疗1月时、6月时与治疗前比较,治疗组收缩压(SBP),舒张压(DBP)、颈内动脉收缩期峰值流速(PSV)、舒张末期流速(EDV)、波动指数(PI)、血流量(VOLUME)有显著差异.治疗6月时与治疗1月时比较,治疗组SBP、DBP、颈内动脉PSV、EDV、PI、VOLUME有显著差异.治疗组与对照组比较,在1月时、6月时SBP、DBP、颈内动脉PSV、EDV、PI、VOLUME有显著差异.对照组观察起始时、1月时、6月时SBP、DBP、颈内动脉PSV、EDV、PI、VOLUME无显著差异.结论 氨氯地平不仅能降低脑梗死后遗症期高血压患者的血压,而且显著改善颈内动脉血流动力学指标,增加颈内动脉血流量.

Abstract:Objective To study effect of amlodipine on internal carotid artery (ICA) hemodynamics in hypertension patient with brain infarction in sequela period. Methods Eighty hypertension patient with brain infarction in sequela period were randomly divided into 2 groups (n=40): therapy group (treated with amlodipine) and control group (treated without drug). Color doppler ultrasonograph was used to measure hemodynamic parameters of ICA in therapy group and control group. Blood pressure is measured with mercury sphygmomanometers. Results The systole blood pressure (SBP) and diastole blood pressure (DBP), the peak systole velocity (PSV), end of diastolic velocity (EDV), pulsatility index (PI), flow volume(VOLUME) of ICA at time of 1 and 6 month therapy were significantly different from ones before therapy in therapy group. The SBP and DBP, the PSV, EDV, PI, VOLUME of ICA in therapy group were significantly different from ones in control group at time of 1 month, 6 month. The SBP and DBP, the PSV, EDV, PI, VOLUME of ICA at time of start, 1 month, 6 month were significantly indifferent from each other in control group. Conclusions Amlodipine can not only drop blood pressure but also improve significantly hemodynamic parameters of ICA to increase flow volume of ICA in hypertension patient with brain infarction in sequela period.
