低位直肠癌保肛手术并应用艾恒化疗的疗效

1999年 11月～ 2 0 0 0年 5月低位直肠癌 (距肛缘 4～ 8cm)保肛手术 41例 ,艾恒组为 2 1例保肛手术并应用艾恒化疗 6个周期 ;对照组为另 2 0例保肛手术后用传统化疗方案 ( 5 FU CF)化疗 6个周期。艾恒组 3年生存率95 2 % ( 2 0 /2 1) ,1例术后 12个月死亡 ,1例术后半年复发改行腹会阴联合直肠癌切除术。化疗毒副反应较轻。对照组术后半年内吻合口复发 4例 ,1年内并肺 肝转移 7例 ,3年生存率 65 0 % ( 13 /2 0 )。初步研究结果提示 ,用艾恒化疗对降低低位直肠癌保肛手术后的复发及转移效果满意 ,应进一步研究

Therapeutic effects of oxaliplatin and sphincter-saving resection combination therapy for distal rectal cancer

From November 1999 to May 2000, fourty-one patients with distal rectal cancer (the distance to sphincter was 4-8 centimeters) were operated on, and sphincters were reserved. In oxaliplatin group, 21 patients were enrolled six-course chemotherapy with oxaliplatim. In control group, other 20 patients were enrolled six-course chemotherapy with 5-FU and CF. The three-year survival rate in oxaliplatin group was 95.2% (20/21),and one patient survived 12 months after operation and one patient accepted Miles' operation six months later for recurrence. The side effect of oxaliplatin was well tolerated. In control group, four patients had recurrence in half a year. Seven patients had lung and liver metastasis in one year. The three-year survival rate was 65.0%(13/20). This result indicates that the chemotherapy with oxaliplatin is very effective for reducing recurrence and metastasis in distal cancer after sphincter-saving resection.
