急性阑尾炎的CT诊断和鉴别诊断

目的：探讨急性阑尾炎的CT表现,并与一些容易混淆的疾病鉴别,提高急性阑尾炎的诊断准确率。方法：回顾分析经手术证实拟诊急性阑尾炎的患者68例分析其临床资料。结果：术前均诊断急性阑尾炎,术后证实符合急性阑尾炎诊断者61例,误诊7例中妇科疾病4例（宫外孕2例,盆腔炎1例,巧克力囊肿1例）,回盲部淋巴结结核1例,盲肠憩室炎1例,急性肠系膜淋巴结炎1例。结论：CT扫描对于急性阑尾炎的诊断有很高的准确性,可以较好地分型,并对病情进行评估。

CT diagnosis and differential diagnosis of acute appendicitis

Objective： To discuss the CT manifestation of acute appendicitis,and identify with diseases which may easily confused with it; increase the diagnose accuracy rates of acute appendicitis. Methods： Review 68 cases of acute appendicitis that confirmed by surgery,age ranged from 12 ~ 85,male 38 cases,female 30 cases. Results： All these 68 cases were diagnosed to be acute appendicitis before surgery,after surgery,61 cases were acute appendicitis,7 cases were misdiagnosed. Which 4 cases were gynecological disease（ ectopic pregnancy 2 cases,pelvic inflammation 1 case,chocolate cyst 1 case）. 1 case was ileocecal lymph node tuberculosis. 1 case was cecal diverticulitis. 1 case was acute mesenteric lymphadenitis. Conclusion： It has a high accuracy rate to diagnose acute appendicitis,it can be clear and definite to type of the acute appendicitis,and it also can be to evaluate the state of an illness.
