283例胰腺癌根治术后患者临床资料和生存分析

目的：探讨胰腺癌根治术后患者临床病理特征与预后的关系。方法回顾性分析行胰腺癌根治手术治疗的、病理确诊为胰腺导管腺癌的患者283例，分析其临床病理特征及生存情况。结果283例患者的中位无瘤生存期（median disease free survival，mDFS）为7．9月，中位总生存期（median overall survival，mOS）为15．0月。多因素分析显示，肿瘤手术切缘状态以及肿瘤TNM分期是影响胰腺癌根治术后患者DFS的独立预后因素（均P＜0.05），肿瘤淋巴结状态以及肿瘤TNM分期是影响胰腺癌根治术后患者OS的独立预后因素（均P＜0．05）。结论对于胰腺癌根治术后患者，手术切缘状态和TNM分期是影响DFS的独立风险因素，TNM分期和肿瘤淋巴结状态是影响OS的独立风险因素。

Survival analysis of 283 cases of pancreatic ductal adenocarcinoma with radical surgery

Objective To investigate the relationship between the clinicopathological features and prognosis of pan-creatic cancer patients after radical surgery.Methods All patients pathologically diagnosed as pancreatic ductal adenocar-cinoma (PDA)and receiving radical surgery were retrospectively analyzed.Their clinicopathological and survival data were examined.Results In two-hundred-and-eighty-three PDA patients with radical operation,the median disease-free survival (mDFS)was 7.9 months,and the median overall survial (mOS)was 15.0 months.Multi-variate analysis determined sur-gical margin and TNMstage as independent risk factors for DFS (P<0.05).Lymph node status and TNMstage are inde-pendent risk factors for OS (P<0.05).Conclusion For PDA patients treated with radical surgery,surgical margin and TNM stage are independent risk factors for DFS,while TNM stage and lymph node status are independent risk factors for OS.
