先天性心脏病合并肺动脉高压患儿血清尿酸、C - 反应蛋白与NT - proBNP的检测及临床意义

摘    要:目的 探讨先天性心脏病合并肺动脉高压患儿血清尿酸(SUA)、C - 反应蛋白(CRP)与氨基末端B型脑钠肽前体(NT - proBNP)的检测及临床意义。方法 通过回顾性分析220例先天性心脏病相关性肺动脉高压患儿的临床病历资料,分析患儿SUA、CRP、NT - proBNP水平与肺动脉高压相关性。结果 心脏病合并肺动脉高压患儿SUA水平随着肺动脉高压程度有逐渐升高的趋势,分别为(0.31±0.04)mmol/L、(0.40±0.08)mmol/L、(0.49±0.08)mmol/L,差异有统计学意义(P<0.05)。CRP随着肺动脉高压程度呈逐渐升高的趋势,分别为(0.59±0.63)mg/L、(1.66±0.28)mg/L、(3.4±0.41)mg/L,差异有统计学意义(P<0.05)。NT - proBNP随着肺动脉高压轻中重度不同,分别为(104.6±15.7)pg/ml、(327.4±20.6)pg/ml、(565.3±45.9)pg/ml,呈逐渐升高的趋势,差异有统计学意义(P<0.05)。经相关性分析,尿酸、C - 反应蛋白、NT - proBNP与肺动脉舒张压、肺动脉收缩压及肺动脉平均压呈正相关。结论 SUA、CRP、NT - proBNP水平与肺动脉压力呈正相关,可间接反映先天性心脏病相关性肺动脉高压患儿的病情严重程度。

Abstract:Objective To investigate the detection and clinical significance of serum uric acid(SUA), C-reactive protein (CRP) and amino terminal B-type brain natriuretic peptide (NT-proBNP) in children with congenital heart disease complicated with pulmonary hypertension. Methods The clinical data of 220 children with congenital heart disease-associated pulmonary hypertension were retrospectively analyzed. The correlation between SUA, CRP and NT-proBNP levels and pulmonary hypertension was analyzed. Results The level of SUA in children with heart disease complicated with pulmonary hypertension increased with the degree of pulmonary hypertension, which were 0.31±0.04 mmol/l, 0.40±0.08 mmol/l, and 0.49±0.08 mmol/l, respectively, the difference was statistically significant(P<0.05). CRP gradually increased with the degree of pulmonary hypertension, which were 0.59±0.63 mg/l, 1.66±0.28 mg/l, and 3.4±0.41 mg/l, respectively, the difference was statistically significant(P<0.05). NT-proBNP,varies with the severity of pulmonary hypertension,showed a trend of 104.6±15.7 pg/mL,327.4±20.6 pg/mL, 565.3±45.9 pg/mL, which was gradually increased with the severity of pulmonary hypertension. Correlation analysis showed that uric acid, C-reactive protein and NT-proBNP were positively correlated with pulmonary artery diastolic blood pressure, pulmonary systolic pressure and mean pulmonary artery pressure. Conclusion The levels of SUA, CRP and NT-proBNP are positively correlated with pulmonary artery pressure, which can indirectly reflect the severity of the disease in children with congenital heart disease-associated pulmonary hypertension.
