逆行交锁髓内钉治疗股骨远端骨折

目的：探讨股骨远端骨折的首选理想治疗方法-应用逆行交锁髓内钉。方法：1998年1月至2002年1月采用逆行交锁髓内钉治疗股骨远端骨折22例。新鲜粉碎性骨折采用工期植骨,陈旧性骨折并膝关节僵硬同时行膝关节松解术。术后早期作CPM锻炼。男13例,女9例,年龄26～45岁。结果：22例均随访11个月以上,骨折愈合时间3～10个月。所有病例无l例出现内固定物松动。断裂。根据Kolment评定标准,优良20例(90％),一般l例(5％),差l例(5％)。结论：逆行交锁髓内钉治疗股骨远端骨折具有固定牢靠,手术时间短,创伤少,操作简便,锁钉定位准确,骨折愈合率高等特点,术后早期行膝关节功能锻炼,利于膝关节功能恢复,所以使用逆行交锁髓内钉是治疗股骨远端骨折的理想方法。

The treatment of distal femoral fractures using retrogate intramedullary interlocking nail

Objective: To investigate the ideal treatment for distal femoral fractures-Application of retrogate intramedullary interlocking nail. Methods: Between January 1998 and January 2002, 22 cases of distal femoral fractures were treated with retrogate intramedullary nails, The primary bone grafting was performed for fresh comminated fracture. Arthrolysis was done simultaneously in every old fracture with knee stiffness. All cases accepted CPM exercises as early as possible after operation. There were 13 males and 9 females, Age's range was 26 to 45 years. Results: All cases were followed up more than 11 months and all fractures united within 3 to 10 months after operation. There were no complications such as nail break related with the fixation. According to the kolment scale, the good and excellent in 20 cases (90%), fair in one (5%) and poor in one (5%). Conclusion: The use of retrogate intramedullary interlocking nail for treatment of the distal femoral fracture, presented with the advantages of simple performance, accurated screw location and high rate of fracture union, therefore it become an ideal therapy for distal femoral fracture.
